Membership Terms
- Monthly membership fee is $200 + 3.75% credit card processing fee. $200 monthly contribution automatically rolls into your account each month.
- Monthly membership fees are automatically charged each month.
- Monthly contributions accumulate and may be applied toward eligible services and products while your membership is active. If you choose to discontinue your membership, credits will remain active for 30 days and must be used within that time.
- Membership benefits are non-transferable.
- Discounts cannot be combined with other practice promotions (monthly specials or package pricing). They can be combined with Alle or Aspire benefits, or other manufacturer rewards.
- Membership may be canceled according to program terms. Six-month minimum commitment; month-to-month after the commitment is over. A 30-day notice is required to cancel.
