At Capizzi MD Cosmetic Surgery & Med Spa, we believe that everyone deserves to look and feel their best. We understand that financing can be a concern for many patients, so we’ve carefully curated a variety of payment options to suit your individual needs.

Third-Party Financing Option
At Capizzi MD, we’re committed to making your dream of looking and feeling your best a reality. We understand that cosmetic procedures can be a significant investment, which is why we offer flexible financing options to suit your needs. Our goal is to help you achieve the results you desire without undue financial stress. Interest rates vary, and start as low as 0% for qualifying patients. Here is a list of the financing options we accept at our practice. If you are curious about any of them, we recommend filling out an application to see what you qualify for:
Alle Rewards
Enjoy the benefits of Alle Rewards! As a participating provider, we accept Alle points, gift cards, and financing options. Earn exclusive perks and rewards when you receive treatments like Botox and filler.
Insurance
Currently, Capizzi MD does not have any partnerships with insurance companies.
Payment & FInancing FAQs
Can I use a gift card or store credit?
Of course! We offer gift cards at our location or they are available for purchase online in our Store.
Do you offer discounts?
We often have quarterly specials and promotions, and will sometimes offer Flash or Monthly promotions for surgical procedures and med spa services. Give us a call to find out about our existing specials!
What if I need to cancel my appointment or procedure?

Do you offer in-house financing options?
We do not offer any in-house financing options at Capizzi MD. All financing options are through a third-party banking institution.
Do you accept insurance?
We do not have any partnerships with health insurance and are considered out-of-network at this time. All procedures would be paid for out-of-pocket and you have the option to pursue reimbursement if you would like. We recommend contacting your health insurance network to ask what their reimbursement process requires.
At Capizzi MD, we understand the importance of clear financial expectations. To ensure a smooth experience, we’ve outlined our payment policy to provide you with information on how to make payments for our services. This policy covers what payment methods we accept, when payment is due, and any associated fees.
Accepted Methods of Payment
We accept all Visa, Mastercard, American Express, and Discover credit cards for payment. You can utilize contactless methods such as ApplePay and SamsungPay or with your physical card. There is a 3% service fee for using a credit card.
We also accept cash, certified checks, or electronic fund transfers for all our services. These methods do not incur any service fees.
You can redeem Alle Rewards and Alle gift cards for payment towards any Allergan service (Botox, filler, etc.) at our office. You can also earn Alle Rewards when you receive any treatment in our office (including non-Allergan services).
Financing Options
We accept CareCredit and Affirm plans for financing any treatment/procedure over $1000. There is an administrative fee of $300 for using a financing option at Capizzi MD. We offer different plans for each, read more below.
Affirm is used largely in the online retailer space at stores such as Amazon. We can utilize your existing credit lines with Affirm for procedures at Capizzi MD. Apply at www.affirm.com/capizzimd to see what you qualify for and what plans you are able to select (this is a soft inquiry). Affirm offers flexibility to our patients and never charges a late fee. CareCredit is commonplace in the healthcare space and their credit lines can be used at medical, orthodontic, or veterinary offices. Through CareCredit, we offer deferred interest plans for up to 12 months, or we offer APR plans for up to 60 months. Apply at www.carecredit.com to see what you qualify for (this is a soft inquiry). Med Spa Appointments
For injectables or facial services, payment for the service is due in full at your appointment time. For some med spa appointments, such as HALO Laser, miraDry, or Morpheus8 treatments, payment is due in full when you book your appointment. When you call to book your appointment, a member of our Patient Care Team will make sure you are aware of the payment policies for your particular appointment.
Surgical Appointments
For surgery, a deposit is due to book your date of surgery. The remainder of the payment is collected at least 2 weeks ahead of your scheduled surgery date, typically at your pre-operative appointment.
